Passports

The Park City Public Library has been designated an official Passport Acceptance Agency by the U.S. Department of State.
All necessary passport application forms are available at the library, and some forms must be filled out prior to submitting the application. 

Those who would like to apply for a passport must provide passport photos at the time they apply. Photos may be acquired at the library for a fee of $15.00. While the library does offer photo service, you may provide your own as long as they meet the requirements.

Please set an appointment for your passport application. You may set an appointment by calling the library phone at (316) 744 - 6318.

Passport hours at the library are:
Monday - Tuesday : 10 a.m. to 4:30 p.m.
Wednesday - Friday: 10 a.m. to 4:30 p.m.
Saturday: 10 a.m. to 11:30 a.m.

It takes 30 minutes to process each passport application per person. If there are multiple applicants a longer time frame will be needed for your appointments. Every person who is applying for a passport MUST be present with their application. This includes children.

Please review the instructions and checklists for the items you will need to bring with you so we can process your passport application.

If you have had a passport and it expires soon, or has expired in the past five years, you may be able to renew your passport by mail. 

If you are eligible for renewal, we cannot process your renewal application here. However, if you would like to bring your renewal form to the library so we can review it with you before you mail it in, we are happy to provide that service.

Each individual passport application must be accompanied by a check or money order made payable to U.S. Department of State for all applicable fees.

ADULT (AGE 16 AND OLDER) FEES:

Passport book — $130
Passport card — $30
Passport book and card — $160
Expedite fee (optional) — $60
Optional overnight return fee, available only for passport book — $19.53

MINOR (AGE 15 AND YOUNGER) FEES:

Passport book — $100
Passport card — $15
Passport book and card — $115
Expedite fee (optional) — $60
Optional overnight return fee, available only for passport book — $19.53

A separate execution fee of $35 per passport application is payable to the Park City Public Library and may be paid by cash, check, money order, or credit/debit card.
